Puppy Plan A... For puppies 12 weeks and younger.

Wellness Examination-  5 visits
DHP-P (includes distemper and parvo)-3 times
Fecal Floatation Tests- at least 3
Fecal Centrifuge Test
Deworming- 2 or more as needed
Bordetella Immunization
Rabies Immunization
Dose of heartworm/flea protection
Toenail Trims (during plan)
Behavior Consultations (during plan)

PLEASE NOTE: Subsequent heartworm /flea prevention, prescriptions,
procedures or tests for other ailments , as well as other immunizations (i.e. Lyme
Disease) are not included in this health plan. However there is no charge for
additional examinations needed for up to 6 months from the start of the
plan.
