At The Family Pet Clinic, we want to help keep your pet in the best health possible. With our full-
service facility, we provide caring top quality treatment to ensure that good health. Being a full-
service facility means a preventive approach in maintaining your pet's health. We provide
preventive medical treatment and consultation, routine checkups, surgical procedures,
radiographs (x-rays), and lab analysis.
You are invited and encouraged to be an active partner in the maintenance of your pet's health
care. We are always happy to provide you with the most current health information relevant to
your pet's health care needs. The professional staff at The Family Pet Clinic continues to
maintain the highest, state-of-the-art professional standards in technological procedures and
preventive/curative medicine.

History
Bartels Pet Clinic was established in 1992 by Dr. Harry Bartels. At that time, Dr. Bartels had
two other clinics in Brecksville and Parma, Ohio. He wanted to bring to Garfield Heights the
superb veterinary care that he already provided in these other cities.
In 1993 Dr. Terri Slezak joined the practice.
In 2000 Dr. Bartels retired and sold the practice to Dr. Richard Thompson. Prior to this, Dr.
Thompson had been practicing for 8 years in Avon Lake, Ohio.
In 2002 we changed our name to The Family Pet Clinic.

Financing Options
Payment Policy
The Family Pet Clinic accepts checks, Mastercard, Visa, Discover, CareCredit and cash for payment.
An estimate will be given at time of recommended treatments, procedures or prescription of medication. A
copy of the estimate will be offered to be sent home and another copy will remain in the pet's file.
The Family Pet Clinic does not offer payment plans for treatments, procedures or prescriptions. In special
situations, a client manager may set up a series of held-checks or automatic debit/credit card payments.
There are specific requirements for these situations.
A minimum deposit of 50% is required for all hospitalized cases at time of admission.
A minimum of $350.00 is required at admission for cases to be seen by a visiting specialist.
The Family Pet Clinic strives to offer your pet the best veterinary care. Please inform us of any cost
concerns and we will help you set up the most appropriate payment method available.
Returned prescriptions that have been opened will receive a 50% credit on your account. Returned
prescriptions that are unopened will receive full credit on your account. It is not the policy of The Family Pet
Clinic to reuse prescriptions whether they are opened or not.
